This is condition-dependent.
Many chronic conditions such as PTSD, fibromyalgia, Chrons, MS, IBS, arthritis, migraines, cancer and other chronic back conditions have an extended timeline on how recent the medical records must be. We always review your records in advance to let you know if the records are sufficient for your approval. If you have records we recommend you fill out a medical release or if you have any in your possession showing a clear diagnosis send them to us or for review so that we can let you know for your specific condition if your records are sufficient or if you will need more current documentation.
You will be required to submit an application online. However, if you are having difficulties with the form or do not have a computer, our staff can assist you. The medical record release form cannot be completed by our staff, but can be mailed to you if necessary.
No. We are not able to complete a medical release form for you because it requires your signature to release your records.
If you are having difficulties with the online form or do not have the computer, we can mail you a paper copy of this form.
No. You will need to fill out the new patient intake form online. We can also provide a paper copy that can be filled out at your appointment.
Note: It is highly recommended that you complete the intake form prior to your appointment. The intake form must be completed before meeting with the doctor.

No. You will not need to provide your Social Security number on the online application. Your number may be required for your state application and for the physicians medical review.
Each and every condition is different. Generally, if your condition should have healed since the case was last received then, yes, additional medical records may be required.
You are required that you send in your medical records in advance. This allows us to prescreen and pre-approve your qualifying condition.
This will also save you a significant amount of time after your appointment and will reduce the visit cost. If your records do not verify your qualifying condition, we will assist you in obtaining the proper medical records.

We will review your records to confirm a diagnosis of a state-approved qualifying condition. It is required by law that the attending physician reviews your medical records.
Once your medical records have been reviewed, your eligibility will be determined. After that, we can continue the process to help you obtain a medical card.
You will be required to submit your

Our clinic provides email updates and will contact you via phone. Without this information, we will not be able to move forward in the process. Your information is completely secure and stored in an encrypted database.
- Complete the physicians application online.
- Submit your medical records via fax, email, or paper mail.
- You may also fill the electronic medical records release form provided after submitting the full physicians application. We can also send one to you via email.
- After we have received your medical records, we will screen them for qualifications and will contact you to inform you of your options.
- Once you are found to be fully qualified, we will schedule an appointment for a simple exam and consultation for filing paperwork with the state.
- You will mail the provided materials to the state and they will issue you your Oregon Medical Marijuana Card.
The state requires that a doctor confirms your qualifying condition and completes a form call the “Attending Physician Statement”.
An “attending physician” means a doctor of medicine (MD) or doctor of osteopathy (DO) licensed under ORS chapter 677 (OAR 333-008-0010(3). MDs and DOs are the physicians licensed under this chapter. The law also specifies that a physician must be licensed in Oregon. The OMMP verifies with the Board of Medical Examiners that each patient’s attending physician has a valid license to practice medicine in Oregon and has no disqualifying restrictions.
According to OAR 333-008-0020(1)(c), a physician must state in writing that the patient has a qualifying debilitating medical condition and that medical marijuana may mitigate the symptoms or effects of that condition.
The OMMP accepts medical records as long as they clearly state the physician is aware that medical marijuana is being used as a treatment and he or she believes the patient may benefit from the use of medical marijuana.
The physician must sign and date the relevant portions of medical records you send to the OMMP. The OMMP contacts each physician during the application process to verify that the patient is under the physician’s care. A signed and dated “Attending Physician’s Statement” and copies of medical records must be current within three months of the date of a person’s new or renewal application.
